 If you have dental or medical insurance that you anticipate to assign benefits for services received, as a courtesy, we will file these services with your insurance provider. This office quotes current fees that are within the usual and customary range of oral and maxillofacial surgery offices in our area. You will be responsible for any deductible, co-pay or non-covered services on the day the service is provided. Final determination for payment towards your claim is then only determined at the time your insurance company processes your claim. Most insurance companies will respond within four to six weeks. Any remaining balance after your insurance has paid is your responsibility. It is important for you to remember that your insurance policy is the agreement that has been set between you, your employer, and your insurance company.
